Job Description:

The Case Aide is part of The Sanctuary residential services team. The primary focus of the Case Aide is to manage/maintain clinical records (paper and electronic), manage client medication needs and documentation, and coordinate client appointments. The ideal candidate for this position will demonstrate strong organizational and communication skills, and knowledge of licensing and ethical standards. Education/Experience/Certifications: High school diploma/GED equivalent or Higher Knowledge and experience with Microsoft and Outlook. Experience working with EMR systems preferred. Experience in a clinical setting preferred. Level One Fingerprint Clearance Card Adult and Pediatric CPR/First Aid Crisis Prevention and Intervention certification (completed in-house) Physical Exam clearing work with no restrictions to position's essential duties Documentation of clearance from communicable diseases (Tuberculosis- updated annually) Valid driver's license and clean driving history Minimum Age Requirement 23 years old Job Requirements:Case Record/Residential Services: Provide initial orientation to campus for new clients. Manage StreetlightUSA assigned MP3 players for clients as part of their coping tools. Upload all client documents into EMR and maintain paper file for original legal documents (certificates, cards, etc). Coordinate all medical appointments with PCH and other general medical and psychiatric providers. Ensure that all medical appointment documentation is received and uploaded to EMR. Manage client records to ensure confidentiality and compliance with licensing, contracts, and accreditation. Coordinate ancillary appointments (on and off-site). Coordinate client visitation on-site. Manage medication documentation and conduct weekly internal audits. Submit medication orders and scripts and ensure delivery or pick-up of medications in a timely manner. Prepare medications and medication documentation for discharge. Close files on EMR after discharge. Coordinate transportation for in house programming and services, as well as track transportation provided by a client's guardian and/or referent/payor. Attend off-site appointments as second staff as needed and/or appropriate. Create and maintain Approved Contact Lists for clients. Provide OJT overview for Case Aide tasks/procedures for new team members. Academic Support: Informs Academic Coordinator (AC) of approved referrals and projected admission dates. Uploads all documents provided by guardian required for enrollment. Assists students with initial account set-up and welcome/orientation video Reviews daily academic reports and relays academic needs to AC Completes weekly academic reports in EMR Collaborates with AC to schedule any off-site academic related appointments Informs AC of upcoming changes and/or discharge dates, AWOLs, etc. Other: Attend mandatory meetings and trainings. Other duties as assigned, which may include standing in for Residential Service Providers during crisis and/or urgent situations.
