an ACPE-accredited School of Pharmacy with a minimum of Doctor of
Pharmacy degree or equivalent experience. Required*Experience** 1
year hospital pharmacy residency or 3 years of clinical pharmacy
experience in a hospital setting. Required*Licenses and
Certifications** Registered Pharmacist (RPh) Licensed to practice
pharmacy in the state of Arizona.Required*Responsibilities: *_*Job
Summary*_Provides expertise in clinical activities relating to
various clinical programs for the pharmacy department in order to
ensure optimal patient outcomes. Facilitates the development,
implementation, and maintenance of programs that promote desirable
patient outcomes through the use of safe, cost-effective drug
therapies. Serves as the expert clinical resource for pharmacy in
the specialty area. Demonstrates ability to work with the patient's
growth and development based upon population groups. Leads general
and specialty direct patient care programs, advanced drug
management services, and all medication use systems throughout the
hospital. Serves as a resource and drug information specialist to
medical, pharmacy, nursing staff and allied healthcare providers.*
Serves in a professional capacity on unit-based decentralized
teams. Monitors patients to assure optimal therapeutic outcomes.*
Monitors drug therapy regimens for contraindications, drug-drug
interactions, drug-food interactions, adverse drug reactions, and
appropriateness of drug and dose. Identifies and intervenes when
problems involving drug therapy arise (i.e. allergies, drug
interactions, therapeutic duplication, and adverse drug
reactions).Functions as a therapeutic consultant and works
collaboratively with providers to provide guidance on appropriate
use of medications, clinical pharmacology, pharmacokinetics,
disease state management, and nutrition. Recommends evidence-based
medication therapy regimens and monitoring plans. Suggests
appropriate, cost-effective therapeutic alternatives to medical
staff as needed.Functions as a therapeutic consultant and works
collaboratively with providers to provide guidance on appropriate
use of medications, clinical pharmacology, pharmacokinetics,
disease state management, and nutrition. Recommends evidence-based
medication therapy regimens and monitoring plans. Suggests
appropriate, cost-effective therapeutic alternatives to medical
staff as needed. Reviews and evaluates the medical literature for
studies supporting the pharmaceutical care and pharmacoeconomic
programs. Reviews the use of targeted medications for compliance
with current pharmacoeconomic programs.Reviews/interprets culture
and susceptibility data for antibiotic appropriateness and
recommends changes, as needed. Performs pharmacokinetic drug-level
consultations per policy guidelines. Assess total parenteral
nutrition (TPN) monitoring and documentation per pharmacy
guidelines and communicates therapeutic recommendations. Provides
patients consultations per pharmacy guidelines and in a timely
manner (i.e. Warfarin, discharge counseling). Promptly perform
patient assessments and participate in the multidisciplinary
clinical rounds on the patient care unit as directed. Sustains
formulary by minimizing non-formulary procurements, utilizing
therapeutic substitution protocols, and promoting rational drug
therapy selection. Reviews medication reconciliation lists and
makes recommendations when necessary.* Develops and implements
evidence-based medication guidelines/protocols. Designs and
implements medication monitoring guidelines and programs for
staff.* Acts as a liaison to the nursing units, in assigned
practice area, in order to communicate pertinent information and to
facilitate communication and increase cooperation. Attends and
actively participates in patient care team meetings by
problem-solving and getting involved in the planning process.*
Develops criteria for medication use evaluations, medication use
indicators, and disease-based evaluations. Collects and analyzes
medication-use data per predefined criteria and formulates
appropriate recommendations for implementation.* Develops the
pharmacy skills of pharmacy staff members by teaching and
exemplifying those skills to the team members; trains and directs
the training of new staff members; participates in staff
educational development encouraging team members to further their
learning in pharmaceutical care to contribute to patient
care/safety; provides educational seminars to pharmacy staff at
least twice a year; provides educational information to physicians
and other Licensed Independent Practitioners (staff), nursing
staff, and other allied health care professionals as needed or
assigned.* Precepts PGY1 and PGY2-Pharmacotherapy residents,
pharmacy students, and contributes to staff development in area of
specialty practice. Serves as the residency program director or
