Our structure is set up to provide Home Health Nurses multiple avenues for support that aren't available at other agencies - a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in office team of QI specialists, telehealth resources, clinical manager and in office triage support. You will enjoy an autonomous practice focusing on medical intervention within the patient's actual home environment. The Home Health space allows for the time to create one on one relationships with your patients and their caregivers and have some flexibility over your schedule that is not possible in the hospital setting. This position offers a flexible schedule, 5/8 or 4/10 with weekend rotation every 4-5 weeks This position will provide 1:1 home care in West Valley with primary areas of coverage in Glendale, Peoria and Phoenix areas. This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members.
