Responsibilities









Terros Health is hiring a Clinical Case Manager at our Olive Health Center in Glendale, AZ. Full-Time, EmployedLocation: Olive Ave.Monday-FridayNo Weekends Education/ Experience Requirements: Full Benefits, Including 401K, and Generous PTO/PST (4+ weeks/yr.) Bilingual Differential Pay Available! $2,000 Sign on Bonus! External Candidates Only Position Summary: Under the Agency's policies and professional requirements, participates as a member of an interdisciplinary clinical team working with individuals diagnosed with a General Mental Health diagnoses. Manages those identified clients who receive medication only. Completes assessments, crisis plans, and individualized service plans. Communicates and documents client progress toward their recovery. Embraces the recovery philosophy and promotes a safe recovery environment for clients. Uses brief clinical interventions, motivational Interviewing, and supportive listening skills to engage and support clients receiving services. Works under the direct supervision of the Clinical Manager. Clinical Case Manager Duties Include:

Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients receiving medication services only.Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate. Coordinates care with a client's family, behavioral and general medical providers and other involved supports including educational, social, judicial, and other State agencies.Participates in clinical staffing's, case conferences, or other meetings, with or without the client or his/her family or other natural support participation.Provides excellent customer service to clients, internal and external provider agencies, State entities, or other stakeholders (e.g. probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed. Qualifications









    • The applicant must have the minimum combination of education and/or experience:
      • A bachelor's degree in the field of behavioral health and any behavioral health experience, or
      • A master's degree or higher in the field of behavioral health.
      • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
      • Excellent oral and written communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
      • Basic understanding of the DSM-V preferred.
      • Proficient in Microsoft Word, Excel, PowerPoint, Outlook. Experience in electronic medical record, NextGen preferred.
      • Ability to work independently, highly organized, and capable of managing multiple ongoing projects.
      • Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy
      • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
      • Must pass a TB Test.
